Why is the only button in the custom block editor OK?

What happened to apply and cancel?

There are two possible causes to this:

  1. You enabled single stepping. Custom block definitions update live with single stepping, and the OK button does nothing but close the window. The other two aren't present, because they wouldn't make sense in that context.
  2. You enabled live coding support. This also makes the apply and cancel buttons disappear, and does the same thing as single stepping, only without showing each step.

You can bring them back by disabling either or both.
