Block Idea for custom hats running

Does it activate all hats in all sprites? Or all of the hats in the caller sprite? Or does it only run the hat the block is under? If not, how do you detect which hat you want to call? What happens if it's in a custom block? What's the difference between once and in loop? I have a lot of questions and don't really see a use case for this.
