Umm. Congratulations on getting the trig right; I always have to work it out painfully because our direction angles aren't counterclockwise from east.
But we already have these blocks! They're CHANGE X BY and CHANGE Y BY.
A minor detail: For the menu input, since your block doesn't accept any inputs other than the two in the menu, you should check the "read only" box in the menu menu.
When the direction is 45, "vertically" moves in the same way as "horizontally". This should be fixed.
